How can I file a claim? (Maya - Cyber Protect)

To file a claim, please submit requirements through the Claim Portal. You may access the claim portal by following the steps below:

1. Go to your Maya App then Services
2. Go to Rewards Section and click Vouchers
3. Look for your claimed and active Cyber Protect Voucher
4. Then click the "Claim Now" Button

Please prepare the following documents before you file for a claim:

Cyber Protect:

• Security Verification of Maya 
• Incident Report
• Screenshot of Exchange Message 
• And any other documents that can support Insurance partner’s  immediate process

Accidental Death/Permanent Disability

• Police/Investigation Report or Affidavit of Witnesses
• Medical Records
• Certified True Copy of Birth Certificate
• Certified True Copy of Death Certificate(Accidental death only)
• Marriage Contract, if married (Optional)
